plan to arrive about one half hour before your first
appointment. This will allow us time to gather the preliminary
information we will need. Please bring your insurance
cards, any medication you are currently taking, and
anything your referring physician may have given to
you. Please remember to bring your referral form if
your insurance is a managed care plan. |
We
suggest that you plan on at least two hours for your initial
visit, although times do vary depending on individual needs.

schedule an appointment at our West shore office, please
call
737-5767. Our regular office hours are Monday through Friday
from 8:00 a.m. to 4:30 p.m. If you have an emergency, call
this same number at any time. The answering service will
contact the physician on call immediately.

the dosage, and the number of your prescription.

practice is affiliated with Holy Spirit Hospital, and Pinnacle
Health System. As mentioned, we also have a unique affiliation
with Johns Hopkins Oncology Center. Our patients have access
to the best available treatments and are also able to participate
in studies using the most promising research medicines.

· Payment in full is due at the time of service
· We accept cash or checks
· We accept Mastercard and Visa upon authorization
We
will be happy to submit insurance claims to your carrier
for covered services and abate payment for 45 days. At that
time, patients are responsible for any outstanding bills.
We participate in many plans. Please check our insurance
page to verify that your plan is included.
If you are covered by a managed care plan, you must obtain
the referral from your primary care physician for all services.
